At Women’s Health & Family Services, we focus on providing comprehensive, compassionate, innovative care to women, men, teens and their families. We are a non-profit, Title X funded organization dedicated to providing primary care services in Clinton County and family planning services in Clinton and Jackson Counties. Prenatal and obstetric services are offered at the Clinton location. Women’s Health & Family Services also provides health education and community outreach within Cedar, Clinton, and Jackson counties.
Two clinic locations are available to meet your needs. Our primary location is in Clinton, Iowa at 2635 Lincoln Way Suite A in the Westgate Medical Plaza building at the west end of Clinton, next to Culver’s. Here patients have access to nurse practitioners, ob/gyn doctors, lab services, and ultrasound diagnostic imaging services all under one roof.
For your convenience, Women’s Health & Family Services also offers services in Maquoketa two days a week. The Maquoketa location is 229 South Main Street, downtown near the local movie theatre.
We will work with you to schedule appointments at a location and time most convenient for you. We strive to get you appointments as quickly as you need to be seen determined by your needs. If you need to reschedule your appointment, we ask that you give our office 24 hours notice. We will contact you a day or two prior to your appointment to confirm the date and time.
Access the Patient Portal and complete the “Pre-Registration” form to become a new patient with Women’s Health & Family Services. Complete basic information to get started. In order to provide the best care for you we want to get your previous medical records for women’s health services from your previous provider. Copies of your most recent year’s annual exam should be sent. More extensive records and medical history are needed for problem visits. Please complete the medical records release form, sign it and send it to your previous provider. Once records are received by Women’s Health & Family Services, we can schedule an appointment. If records are not received and duplicate services are performed and not covered by your insurance, you may be responsible for the charges.
If you have health insurance, we advise that you check with your insurance carrier before your appointment to verify your coverage and benefits. Many benefit plans have high deductibles or special requirements including:
1. Network providers; make sure Women’s Health & Family Services providers are in your network. You can still be seen at Women’s Health & Family Services if the provider is not in network, however you may be responsible for more of the cost of the services.
2. Deductible and co-pay amounts; see if you have any that may apply to your type of visit.
3. Pre-authorization requirement, if any. If you have a pre-authorization requirement it is important for you and Women’s Health & Family Services to work with your insurance carrier to get approval for your services. One of our staff will help you obtain pre-authorization with your insurance and if needed work out payment arrangements with you for the services.
